Description:

Neratinib is an oral tyrosine kinase inhibitor used primarily in the extended adjuvant treatment of early-stage HER2-positive breast cancer following trastuzumab-based therapy. It works by irreversibly binding to the HER1, HER2, and HER4 receptors, blocking their signaling pathways and thus inhibiting cancer cell growth.

Neratinib is specifically used to reduce the risk of cancer recurrence after initial treatment in patients with HER2-positive breast cancer.

Dosage Form:

• Oral tablets

Typical Dosage:

• The usual recommended dose of Neratinib (Hernix) is 240 mg once daily (usually taken as 4 tablets of 60 mg each).

• It is often prescribed for 1 year (12 months) following completion of trastuzumab therapy.

• Neratinib should be taken with food to improve absorption.

• Dose adjustments may be necessary depending on side effects, especially diarrhea, which is a common adverse effect. Prophylactic anti-diarrheal treatment is often recommended when starting Neratinib.

Important Notes:

• Patients should be monitored for liver function tests during treatment.

• Diarrhea management is crucial; loperamide or other anti-diarrheal medications are typically prescribed prophylactically or at the first sign of diarrhea.
